Job Brief
As a Hospice Home Care Social Worker, you will provide essential psychosocial support to individuals, families, and groups facing chronic, acute, or terminal illnesses. This role involves advising family caregivers, offering counseling and information to patients, and facilitating referrals to appropriate services. You will also be responsible for case and care management interventions aimed at promoting health, preventing disease, and addressing barriers to accessing healthcare. The ideal candidate will possess strong communication, empathy, and organizational skills, with relevant experience in hospice or palliative care settings.
Responsibilities
- Conduct comprehensive assessments of patients and families to identify psychosocial needs.
- Provide emotional support and counseling to patients and their families during end-of-life care.
- Advise and educate family caregivers on coping strategies and available resources.
- Facilitate referrals to community resources and additional healthcare services as needed.
- Collaborate with interdisciplinary teams to develop and implement patient-centered care plans.
- Monitor and evaluate patient progress and adjust care plans accordingly.
- Document patient interactions and maintain accurate records in compliance with regulations.
- Participate in case conferences and contribute to quality improvement initiatives.
Requirements
- Master’s degree in Social Work (MSW) from an accredited institution.
- Current licensure as a Social Worker (LCSW or equivalent) in the applicable state.
- Experience in hospice, palliative care, or similar healthcare settings preferred.
- Strong knowledge of community resources and healthcare systems.
- Excellent communication, interpersonal, and counseling skills.
- Ability to work independently and as part of an interdisciplinary team.
- Proficient in electronic health record (EHR) systems and documentation practices.
