Job Brief
The Coordinator of Rehabilitation Services plays a vital role in enhancing the independence and employability of individuals facing personal, social, and vocational challenges due to physical or mental health issues, disabilities, or life stressors. Responsibilities
- Conduct comprehensive assessments to identify client needs and challenges.
- Develop and implement personalized rehabilitation plans incorporating counseling, training, and job placement services.
- Coordinate and facilitate rehabilitation activities in collaboration with care and treatment facility staff.
- Monitor and evaluate client progress, adjusting programs as necessary to optimize outcomes.
- Provide one-on-one counseling to help clients overcome barriers to independence and employment.
- Maintain accurate records and documentation of client progress and program effectiveness.
- Advocate for client needs and connect them with community resources and support services.
- Stay updated on industry trends and best practices in rehabilitation services and counseling.
Requirements
- Bachelor’s degree in Rehabilitation Counseling, Social Work, Psychology, or a related field.
- Experience in counseling, rehabilitation services, or social services preferred.
- Knowledge of rehabilitation techniques, vocational training, and job placement strategies.
- Strong interpersonal and communication skills, with the ability to build rapport with clients.
- Ability to work collaboratively with multidisciplinary teams in care settings.
- Familiarity with assessment tools and program evaluation methods.
- Certification in Rehabilitation Counseling (CRC) or related credential is a plus.
- Proficient in using technology and software for client management and documentation.
