Training attendance sheet
In short
A training sign in sheet records who attended a training session, when they arrived and left, and whether they completed it, along with the course, date, trainer and materials used. This template adds time in and out, assessment results and a trainer certification so the sheet can serve as a compliance training record, not just a headcount.

Training Attendance Sheet
Each attendee signs in on arrival and records the time out when they leave. Print your name clearly. Your signature confirms you attended the session described below. The trainer completes the session details and certification and returns the sheet to [HR / L&D] within [2] business days.

How to use this template
- Fill in the session details before the class starts, including the version of the materials you will use.
- Have each attendee print their name, employee ID and time in, and sign as they arrive rather than at the end.
- Record time out and mark anyone who left early or missed part of the session as partial attendance.
- Add assessment scores or pass/fail results where the course includes a test.
- Sign the trainer certification, list no-shows for a make-up session and return the sheet to HR or L&D within two business days.
- Enter completions in your LMS or training matrix and store the original sheet for your retention period.
More than a headcount
Most sign in sheets are a list of names and signatures on a clipboard. That is enough to count heads for lunch, and not much else. If the session was required safety, harassment or compliance training, the sheet may be your only proof that a specific person received specific content on a specific date, and a bare signature list proves less than you think. Which version of the policy did they hear? Did they stay for the whole hour, or sign in and leave at minute ten?
The template adds the fields that make the sheet stand up later: course version, topics covered, trainer and qualification, time in and out, assessment result and a trainer certification. For ongoing tracking across many courses, roll the completions up into a training matrix.
When the record carries legal weight
Safety training. Some OSHA standards spell out training records. For powered industrial trucks, for example, the employer must certify that each operator has been trained and evaluated, and the certification must include the operator's name, the date of training, the date of evaluation and the identity of the person who did the training or evaluation. A sign in sheet with those fields and the evaluator's signature goes most of the way.
Harassment prevention training. Several states mandate it. California requires employers with five or more employees to provide it every two years (two hours for supervisors, one hour for other employees) and its regulations call for keeping training records for at least two years. New York requires annual training. Record attendance per person, not per session.
Pay. Under the FLSA, time spent in training is generally working time for non-exempt employees unless it is outside normal hours, truly voluntary, not directly related to the job and involves no productive work. Required training almost never meets all four, so the time in and out on this sheet should flow into timekeeping.
Requirements vary by state, industry and country. Check the rules for each training program, especially in healthcare, transportation and financial services.
New hire classes and volume training
In a contact center onboarding a 30-person class every month, the sign in sheet is a daily attendance record for two to six weeks of classroom training and nesting. Use one sheet per day, not one per course. That gives you the data to spot the pattern behind training attrition: absences on day three, a cluster of early departures after the systems module, a trainer whose classes lose more people than others.
For virtual sessions, export the platform's attendance report with join and leave times and attach it to the sheet, rather than relying on a chat-box roll call. Link completions back to the new hire training checklist so supervisors know which modules each agent has finished before they take live calls. If no-shows are a problem, send a training reminder email the day before with the start time and location.
Common mistakes
- Passing the sheet around at the end, so people sign for colleagues who already left.
- No course version, so you cannot show which policy content was delivered.
- Paper sheets that never get entered in the LMS and are lost when the trainer changes roles.
- Not recording time, so partial attendance counts as completion.
- Forgetting to pay non-exempt employees for required training outside their normal shift.
Frequently asked questions
What should a training attendance form include?
Course title and version, date, time, location, trainer, topics covered, each attendee's printed name, ID, time in and out and signature, assessment results where relevant, a list of no-shows and a trainer certification.
What is a training register template?
It is another name for a training attendance sheet or log, used to record who attended which training and when. Some organizations use "register" for a running log across many sessions rather than a sheet for a single class.
How long should training sign in sheets be kept?
It depends on the training. Some laws set minimums, such as California's two-year requirement for harassment training records, and some OSHA standards have their own rules. Many employers keep training records for the length of employment plus several years.
Can a class sign-in sheet be digital?
Yes. Tablets, QR-code check-ins and virtual meeting attendance reports all work, provided they capture the same information, identify each person reliably and are kept in a way you can retrieve later.
Is training time paid for hourly employees?
Usually. Under federal law, training is working time unless it is outside normal hours, voluntary, not directly job-related and involves no productive work. Required training rarely meets all four conditions.
