Return to work form

By TapTalent Editorial TeamUpdated

In short

A return to work form documents an employee's return after an absence: the dates and type of leave, any medical clearance or fitness for duty certification, work restrictions, accommodations to review, and notes from a short return to work interview. This template handles both routine sick days and returns from extended medical or FMLA leave.

Return to Work Form

Complete Part A for every return. Complete Part B only when the absence was for the employee's own serious health condition, a work injury or another reason where the company's written policy requires medical clearance. Medical documents go to HR and are stored separately from the personnel file. Do not ask the employee for a diagnosis.

How to use this template

  1. Decide from your written policy whether this return needs Part A only or a medical clearance in Part B, and apply that rule the same way for everyone in similar situations.
  2. For FMLA leave, tell the employee in the designation notice if fitness for duty certification will be required, and attach the job's essential functions if you want the provider to address them.
  3. Hold the return to work conversation on the first shift back; it should take ten minutes, not an hour.
  4. Record restrictions exactly as the provider wrote them and route anything you cannot accommodate to HR the same day.
  5. Get signatures, send medical pages to the confidential medical file and file Part A with attendance records.
  6. Set a review date for any temporary restrictions and put it in the manager's calendar.

Two jobs, one form

Return to work forms do two different things, and mixing them up causes most of the problems. The first is a simple welcome-back record after an ordinary absence: dates, reason category, a short conversation about whether the person is ready and what they missed. The second is a medical clearance after a longer leave or an injury, where you need to know the employee can safely do the job.

The template separates them. Part A works for every return, including a single sick day. Part B is used only when your written policy requires a medical release. Asking for a doctor's note after every one-day absence is a quick way to frustrate good employees and, in many places, to break a paid sick leave law.

The return to work interview

A short, consistent conversation on the first day back does more for attendance than any points policy. It tells the employee their absence was noticed, gives them a chance to raise anything that would make a repeat absence less likely, and catches early signs of a longer-term health issue or a need for accommodation.

Keep it short and supportive. Ask how they are, whether they are ready for normal duties, and what they need to know about what changed while they were out. For a bank of questions that stay on the right side of medical privacy, see our return to work interview questions. Do not ask what their illness was. If you send a note to confirm the return date beforehand, the return to work email templates cover the common situations.

Fitness for duty and the law

FMLA. Under the FMLA regulations, an employer may require a fitness for duty certification before restoring an employee who took leave for their own serious health condition, but only if it has a uniformly applied policy for similarly situated employees and it told the employee in the designation notice. The certification can address the employee's ability to perform essential functions only if a list of those functions was provided with the designation notice. For intermittent leave, a certification can be required at most once every 30 days, and only when reasonable safety concerns exist. Use the FMLA leave request form at the start of leave so these steps are not missed.

ADA. Medical questions and exams of current employees must be job-related and consistent with business necessity. Restrictions trigger the interactive process; "no restrictions" policies that refuse to return anyone until fully healed are a well-known source of disability discrimination claims.

Paid sick leave laws. Many state and city sick leave laws limit when you can ask for documentation, often only after more than three consecutive workdays, and do not allow you to require details of the illness. Align the form with your sick leave policy.

Rules vary by state and country, so check local requirements, especially for work injuries, where workers' compensation rules also apply.

Using it in shift-based operations

In a contact center or warehouse running 24/7, the employee may come back on a night shift when HR is not on site. Give team leads a mobile version of Part A, train them on what not to ask, and route Part B straight to HR rather than letting a supervisor read medical details. A returning agent who needs a modified schedule should not sit at home for three days because the clearance is stuck in someone's inbox.

Track returns alongside absence data. If one team has twice the rate of returns needing restrictions, that is a workstation or workload signal worth investigating.

Common mistakes

  • Asking for a diagnosis or storing doctor's notes in the personnel file.
  • Requiring medical clearance from some employees but not others in the same situation.
  • Refusing to return someone until they have "no restrictions".
  • Delaying the return while HR chases a clarification from the provider.
  • Skipping the conversation for short absences, which is where patterns first show up.

Frequently asked questions

What is a fitness for duty form?

It is a certification from a health care provider confirming that an employee can return to work and perform their job, sometimes with listed restrictions. Under the FMLA it can be required only under a uniformly applied policy that the employee was told about in the designation notice.

What should a return to work interview form include?

Absence dates and category, whether the call-out procedure was followed, whether the employee is ready for normal duties, any support needed, work updates shared, a follow-up date and signatures. It should not ask for a diagnosis.

Is a return to work letter the same as a return to work form?

No. A return to work letter is usually sent to the employee to confirm the date, schedule and any conditions of the return. The form records what happened when they came back, including any clearance and restrictions.

Can an employer require a doctor's note to return to work after a sick day?

It depends on where the employee works. Many paid sick leave laws limit documentation requests to absences longer than a few consecutive days and bar requiring details about the illness. Check the state and city rules and your own policy.

What if the employee returns with restrictions?

Review whether the restrictions can be accommodated in the current role, start the interactive process if needed, agree any modified duties in writing and set a review date. Do not send the employee home simply because restrictions exist.
