Job Brief
As a Neonatal Social Worker, you will provide essential psychosocial support to individuals, families, and groups facing chronic, acute, or terminal illnesses. Your primary responsibilities will include advising family caregivers, offering counseling and information to patients, and making referrals to additional services. You will also engage in case and care management interventions that promote health, prevent disease, and address barriers to accessing healthcare. The ideal candidate will possess a strong understanding of neonatal health issues and demonstrate excellent communication and empathy skills.
Responsibilities
- Conduct assessments of patients’ psychosocial needs and develop personalized care plans.
- Provide emotional support and counseling to patients and their families during challenging medical situations.
- Facilitate communication between patients, families, and healthcare teams to ensure coordinated care.
- Advise and educate family caregivers on coping strategies and available resources.
- Assist patients in navigating healthcare systems and accessing community resources.
- Implement case management strategies to address barriers to healthcare access.
- Maintain accurate and confidential patient records in compliance with legal and ethical standards.
- Participate in interdisciplinary team meetings to promote patient-centered care.
Requirements
- Master’s degree in Social Work (MSW) from an accredited program.
- Current licensure as a Clinical Social Worker (LCSW) or equivalent certification.
- Experience working in a neonatal or pediatric healthcare setting preferred.
- Strong knowledge of psychosocial issues related to neonatal health and family dynamics.
- Excellent interpersonal, communication, and advocacy skills.
- Ability to work collaboratively with healthcare professionals and community organizations.
- Proficiency in case management software and electronic health records (EHR).
- Valid driver’s license and willingness to travel as needed for home visits or community outreach.
