Job Brief
The Community Health Nurse plays a vital role in assessing patient health problems and needs within the community. Responsibilities
- Conduct thorough health assessments to identify patient needs and health problems.
- Develop, implement, and evaluate individualized nursing care plans.
- Administer nursing care to patients, including monitoring vital signs and administering medications.
- Maintain accurate and detailed medical records for all patients.
- Advise patients on health maintenance, disease prevention, and health education.
- Provide case management services to coordinate patient care and resources.
- Participate in community outreach programs to promote health awareness.
- Collaborate with healthcare teams to provide comprehensive patient care.
Requirements
- Active nursing license or registration in the relevant state or jurisdiction.
- Bachelor’s degree in Nursing (BSN) or higher preferred.
- Experience in community health nursing or public health settings.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal skills.
- Knowledge of health promotion, disease prevention, and community resources.
- Basic computer skills for maintaining electronic medical records.
- Certification in Basic Life Support (BLS) or Advanced Cardiac Life Support (ACLS) may be required.
