Job Brief
The Palliative Care Nurse Practitioner plays a vital role in the healthcare team, focusing on providing comprehensive care for patients with serious, chronic, or life-limiting illnesses. Responsibilities
- Conduct thorough patient assessments, including physical exams and health histories, to diagnose acute and chronic illnesses.
- Develop and implement individualized care plans that align with patient goals and healthcare needs.
- Order, perform, and interpret diagnostic tests, such as laboratory tests and imaging studies.
- Prescribe and manage medications, ensuring safety and efficacy in treatment protocols.
- Educate patients and families on disease processes, treatment options, and palliative care principles.
- Collaborate with physicians, nurses, and other healthcare professionals to coordinate comprehensive care.
- Advocate for patients’ rights and preferences in care decisions and treatment choices.
- Maintain accurate and up-to-date patient records in accordance with regulatory standards.
Requirements
- Master’s degree in nursing with a specialization in palliative care or a related field.
- Current Nurse Practitioner (NP) licensure and certification in Palliative Care or similar specialty.
- Minimum of 2 years of clinical experience in a healthcare setting, preferably in palliative or hospice care.
- Strong knowledge of pain management, symptom control, and end-of-life care best practices.
- Excellent communication and interpersonal skills to effectively interact with patients and families.
- Proficient in electronic health records (EHR) and documentation standards.
- Ability to work independently and as part of a collaborative healthcare team.
