Job Brief
As a Hospice Social Worker, you will be responsible for providing essential psychosocial support to individuals and families coping with chronic, acute, or terminal illnesses. Your role will include advising family caregivers, delivering counseling, and facilitating access to necessary resources and services. Ideal candidates will have experience in case management and a compassionate approach to enhancing the quality of life for patients and their families during challenging times.
Responsibilities
- Conduct comprehensive psychosocial assessments to identify patients’ emotional, social, and environmental needs.
- Provide counseling and support to patients and their families to help them cope with the impact of illness.
- Advise and educate family caregivers on coping strategies and available community resources.
- Coordinate referrals to other healthcare services and community resources to ensure comprehensive care.
- Collaborate with interdisciplinary teams to develop and implement patient care plans.
- Facilitate support groups and educational workshops for patients and families.
- Monitor and evaluate the effectiveness of interventions and adjust care plans as needed.
- Maintain accurate and timely documentation of patient interactions and care plans in compliance with regulations.
Requirements
- Master’s degree in Social Work (MSW) from an accredited institution.
- Current state licensure as a Licensed Clinical Social Worker (LCSW) or equivalent.
- Proven experience in hospice, palliative care, or a related field.
- Strong knowledge of healthcare systems and community resources.
- Excellent communication and interpersonal skills.
- Ability to work independently and collaboratively within a multidisciplinary team.
- Compassionate and empathetic approach to patient and family care.
- Proficiency in case management and care coordination practices.
