Job Brief
We are seeking a dedicated Family Health Nurse Practitioner to join our healthcare team. In this role, you will be responsible for diagnosing and treating acute, episodic, and chronic illnesses, focusing on health promotion and disease prevention. You will work independently or collaboratively with other healthcare professionals, order and interpret diagnostic tests such as lab work and X-rays, and prescribe medications. Responsibilities
- Conduct comprehensive health assessments to diagnose and manage patient conditions.
- Develop and implement individualized care plans based on patient needs and preferences.
- Order, perform, and interpret diagnostic tests, including laboratory and imaging studies.
- Prescribe appropriate medications and treatment regimens based on clinical guidelines.
- Educate patients and families on health promotion, disease prevention, and management of chronic conditions.
- Collaborate with interdisciplinary healthcare teams to deliver coordinated patient care.
- Maintain accurate and up-to-date patient records in compliance with regulations.
- Stay current with advancements in family health practices and participate in continuous professional development.
Requirements
- Master’s degree in Nursing with a specialty in Family Health or related field.
- Current state licensure as a Nurse Practitioner and Registered Nurse.
- Certification as a Family Nurse Practitioner (FNP) from an accredited body.
- Strong clinical assessment and diagnostic skills.
- Proficiency in electronic health record (EHR) systems.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a healthcare team.
- Knowledge of evidence-based practices and clinical guidelines.
