Job Brief
As a Community Health Worker (CHW), you will play a vital role in promoting health within the community by assisting individuals in adopting healthy behaviors and effectively advocating for their health needs. This position requires a proactive approach in serving as a liaison between community residents, healthcare providers, and social service agencies. You will implement programs designed to enhance individual and community health, deliver preventive health services, and collect data to identify community health needs.
Responsibilities
- Promote healthy behaviors and wellness initiatives within the community.
- Advocate for individuals’ health needs and facilitate communication with healthcare providers.
- Implement health programs that aim to improve community health outcomes.
- Deliver preventive health services, including screenings for blood pressure, glaucoma, and hearing.
- Collect and analyze data to assess community health needs and identify areas for improvement.
- Educate community members about health resources and available services.
- Collaborate with local organizations to enhance health initiatives and outreach.
- Maintain accurate records of services provided and community engagement efforts.
Requirements
- High school diploma or equivalent; relevant certification or degree in public health or social work is a plus.
- Experience as a Community Health Worker or in a similar role preferred.
- Strong understanding of community health issues and available resources.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a multidisciplinary team.
- Proficient in data collection and analysis methods.
- Knowledge of health education principles and preventive services.
- Fluency in multiple languages is advantageous.
