Job Brief

The Renal Social Worker provides essential psychosocial support to individuals, families, and groups dealing with chronic, acute, or terminal renal illnesses. This role involves advising family caregivers, delivering information and counseling to patients, and facilitating referrals to additional services. The ideal candidate will possess strong case management skills, focusing on health promotion, disease prevention, and addressing barriers to healthcare access. A background in social work with a focus on renal care is preferred.

Responsibilities

  • Conduct comprehensive assessments of patients’ psychosocial needs related to renal health.
  • Develop and implement individualized care plans that address specific patient and family needs.
  • Provide counseling and support to patients and their families coping with chronic and terminal illnesses.
  • Educate patients about renal disease management, treatment options, and community resources.

Requirements

  • Master’s degree in Social Work (MSW) from an accredited institution.

  • Experience in healthcare or social services, preferably in renal care or chronic illness management.
  • Strong knowledge of community resources and healthcare systems.
  • Excellent communication and interpersonal skills for effective patient interaction.
